In some cases, an approval is needed from your health plan before some health care services will be covered. This is called prior authorization.

Your doctor is responsible for getting a prior authorization. They will provide us with the information needed. If a prior authorization is approved, those services will be covered by your health plan. If a prior authorization is denied, you may be responsible for the cost of those services.

A letter will be mailed to you after we review the request for a prior authorization. The letter will tell you if the request was approved or denied. If the prior authorization request was denied, the letter will explain why it was denied.
